She was made from a Fluttershy Body and a Pinkie Pie head. Full Body Repaint which has been sealed to protect against chips. Her little tongue has been sculpted from apoxie and sealed with triple gloss sealent for a wet look. Hair is dollyhair.com's Lemonade.

She's for sale, $35 plus shipping, paypal only.

for me as a total FiM noob, is she a real character or did someone make her up?
 
She was an animation mistake in the background of the pilot episode, but the fans latched onto her (see Derpy Hooves [MLP:FiM]) so she keeps showing up. It's gotten to the point where people actively hunt for cameos of her in the show. XDD
